"The startling secrets begin on the book's cover, where there is a hidden image only the most careful observers will detect."
What is the hidden image?

The hidden image on the cover is made by the back of Beatrice's hair...it creates a cut-out image of a smaller head that looks just like hers. What this means is up for interpretation, of course. It could be her off-spring....(Violet) or not.

Yes, I just noticed it! You mean the ribbons which are behind the hidden image, right? The ones which seem to form the hidden girl's hair and ribbon?

The first head is Beatrice the older, the one LS wnted to marry. i think that the smaller head is Beatrice's the younger. I mean, the whole book is about these two Beatrices, and I don't think that Violet has anything to do with it.
