The odd thing about Olaf and the evil side of VFD, is that their fires never seem to kill anyone.
The carnival fire in Book 9 didn't kill anyone. The Headquarters fire in Book 10 didn't kill anyone. And the fire in Book 12 was technically the Baudelaire kids' idea. It wasn't Olaf's original plan to burn the hotel. The hospital fire in Book 8 may have killed some people, but Olaf shouted over the intercom multiple times, that people should evacuate the hospital or rescue patients in the wards. He didn't have to warn people like that.
I know that in Book 13, Ishmael says that Olaf locked him in a cage and killed his parents in a fire, but Olaf directly says he didn't do it. Olaf's eyes have the creepy sparkle when he denies it, but he could've been enjoying provoking Ishmael. Olaf has always admitted that he's a murderer and arsonist, so if he says he didn't set the fire, I don't see why he wouldn't be telling the truth.
Olaf is evil and psycho, but he's the kind of person who would have bragged to the Quagmires and Baudelaires that he killed their parents...
Is it possible that the parents dying was an accident, and that the Baudelaire and Quagmire kids just got lucky enough to survive?

There is no evidence regarding the cause of the Baudelaire fire.
Regarding the Quagmire fire, Quigley testifies that "I heard a shattering of glass, and people shouting." (TSS p. 159) This can be related to his testimony regarding the Montgomery fire: "someone had thrown a torch through the glass of the ceiling, starting a fire in the library." (TSS p. 170) The shattering of glass in the Quagmire fire may have similarly been caused by the violent projection of a torch into the mansion through a glass window.

The fire in THH was not an accident, and it killed at least one person, Olaf's large associate. So some of the fires do kill people.
